Inilabas ng Philippine Postal Corporation (PHLPost) ang requirements para sa application, maging ang renewal ng Postal IDs.
Ang mga aplikante ay kailangan ng mga sumusunod:
1. Kopya ng duly-accomplished PID application form
2. Proof of Identity – orihinal na kopya at (1) photocopy ng alin mang sumusunod na dokumento:
– National ID
– National ID (Digitized and/or paper)
– PSA Birth Certificate o Local Civil Registry na may resibong inisyu sa loob ng 6 na buwan
– GSIS UMID Card
– SSS UMID Card
– Driver’s License
– Passport
– Marriage Certificate for Married Women
3. Proof of Address – orihinal na copy at (1) photocopy ng alin mang sumusunod na dokumento:
– Barangay Certificate of Residency issued within three (3 months) prior to PID application
– Barangay Residence ID
– Certified True Copy of Lease
– Certified True Copy of Titles issued by the Land Registration Authority (LRA)
– Certified True Copy of Real Estate Tax Receipt
– Bank Statement
– Credit Card Statement
– School Billing Statement
– Utility Bill (cable, electric, internet, landline, telephone, water)
Para naman sa mga renewal applicants o nawala ang valid Postal ID, kailangan ng mga sumusunod:
For applicants for renewal or loss of their valid postal ID, they should provide the following:
1. Kopya ng duly-accomplished PID application form
2. For renewal – expired postal ID; para sa loss valid postal ID – magsumite ng duly-notarized affidavit of loss
3. Proof of Identity – orihinal na kopya and (1) photocopy ng alin mang sumusunod na dokumento:
– National ID
– National ID (Digitized and/or paper)
– Birth Certificate from PSA or Local Civil Registry with receipt issued within 6 months
– GSIS UMID Card
– SSS UMID Card
– Driver’s License
– Passport
– Marriage Certificate for Married Women
4. Proof of address – orihinal na kopya at (1) photocopy ng alin mang sumusunod na dokumento:
– Barangay Certificate of Residency issued within three (3 months) prior to PID application
